Halo 4D Imaging – Terms & Conditions

 

 

By booking an appointment with Halo 4D Imaging, you agree to the following terms and conditions:

 

 

1. Nature of Services

 

 

Halo 4D Imaging provides elective, non-diagnostic 3D/4D ultrasound services for entertainment and bonding purposes only.

These sessions are not intended to replace medical care, prenatal visits, or diagnostic ultrasounds performed by your healthcare provider.

 

 

 

2. Eligibility

 

 

Clients must already be receiving prenatal care from a licensed healthcare provider.

By booking, you confirm that you are under medical care for your pregnancy.

 

 

 

3. Booking & Payment

 

 

A valid appointment is required prior to services being rendered.

Payment is due at the time of your appointment unless otherwise stated.

 

 

 

4. Travel Fees

 

 

Halo 4D Imaging is a mobile, in-home service.

A travel fee of $1.50 per mile may apply outside of designated travel days.

Discounted travel rates may be offered when multiple clients are booked in the same area.

 

 

 

5. Cancellation & Rescheduling

 

 

We kindly ask for at least 24 hours notice for cancellations or rescheduling.

Failure to provide proper notice may result in a cancellation fee or loss of deposit (if applicable).

 

 

 

6. Image Quality Disclaimer

 

 

Image quality can vary depending on several factors including, but not limited to:

 

• Baby’s position

• Gestational age

• Placenta placement

• Maternal body composition

 

 

While we do our absolute best to capture beautiful images, we cannot guarantee specific results.

 

 

 

7. Gender Determination

 

 

Gender determination is offered as an optional service but is not guaranteed.

Halo 4D Imaging is not responsible for incorrect gender determination.

 

 

 

8. Refund Policy

 

 

Due to the nature of the service, all sales are final.

No refunds will be issued once services have been rendered.

 

 

 

9. Safety

 

 

All ultrasounds are performed using safe, non-invasive equipment and follow recommended guidelines for elective imaging.

 

 

 

10. Media Release

 

 

By booking, you grant Halo 4D Imaging permission to use images or videos from your session for marketing and promotional purposes, unless otherwise requested in writing.

 

 

 

11. Limitation of Liability

 

 

Halo 4D Imaging is not liable for any medical concerns, outcomes, or conditions related to your pregnancy.

If any concerns arise during your session, you will be advised to contact your healthcare provider
